Eliseo J. Pérez-Stable, M.D. is Professor of Medicine at the University of California, San Francisco (UCSF) School of Medicine. He holds degrees from the University of Miami (B.A. and M.D.), trained in primary care general internal medicine at UCSF, and completed a Henry J. Kaiser Family Foundation fellowship in general internal medicine before being appointed Assistant Professor at UCSF in July 1983. Dr. Pérez-Stable's research has focused on risk factor reduction interventions for Latino populations and health care disparities by race and ethnicity.

Dr. Pérez-Stable was the Principal Investigator of the Hispanic Smoking Cessation Research Project (Programa Latino Para Dejar de Fumar) which was a community based intervention to decrease nicotine dependence in San Francisco Latinos funded by the National Cancer Institute (NCI) from 1985-1996. He also was Leader of a project in the Pathways to Cancer Screening in Four Ethnic Groups NCI funded program project that included a community based intervention targeting Latina women in San Francisco to promote screening procedures for early detection of breast and cervical cancer. Dr. Pérez-Stable was also the Co-PI for San Francisco in the National Hispanic Leadership Initiative on Cancer (En Acción). En Acción is a community based intervention to promote smoking cessation, decreased alcohol consumption, increased use of recommended cancer screening tests, and changes in dietary habits to promote a healthier lifestyle. These projects are an extension of prior work that compared knowledge, beliefs, attitudes and practices of Latinos and Anglos in Alameda and San Francisco Counties in order to develop cancer prevention interventions promoting cancer screening tests. As a result of these programs three booklets for Spanish speaking population were produced. The Guia para Dejar de Fumar which has been published by the NCI, a guide for women to promote use of breast and cervical cancer screening tests (La Mujer y El Cancer) and a general information booklet on cancer prevention and control.

Dr. Pérez-Stable has studied Sociocultural Barriers Between Latino Patients and their Anglo Physicians by evaluating the effect of language ability on medical outcomes and culture specific barriers to communication. This work led to the observation that Spanish monolingual Latino patients felt better with bilingual clinicians than with clinicians using interpreters. Dr. Pérez-Stable is Director of the UCSF Medical Effectiveness Research Center for Diverse Populations (MERC) and Director of the Center for Aging in Diverse Communities (CADC). MERC is focusing on health and health care disparities in African-American, Asian-American, and Latino populations with a special emphasis on cancer and reproductive health and working groups on methodological areas related to ascertainment of race or ethnicity, and quality of care. In an extension of the quality of care work, CADC and MERC help with training of minority scientists by focusing on mentoring relationships with established faculty. CADC funds pilot studies for minority investigators, has established a network of community based organizations working in minority communities, and is evaluating the appropriateness of using measures in diverse groups. Dr. Pérez-Stable follows a panel of patients in general internal medicine, and teaches residents and students in primary care internal medicine every week. He is a research mentor for residents, fellows, and faculty at UCSF and other institutions.

Current Research Activities, 2004-2007

My research is focused on health and health care disparities by race and ethnicity with a special emphasis on cancer prevention and cigarette smoking cessation among Latino populations. In 1993, I co-founded the Medical Effectiveness Research Center for Diverse Populations (MERC) in collaboration with A. Eugene Washington, MD and was Co-Director until July 2005 when I became Director. MERC is made up by a team of multidisciplinary investigators focused on health disparities by race/ethnicity with a special emphasis on cancer prevention, reproductive health, patient-doctor communication, and aging. My work has elucidated the causes of health disparities in Latinos and tested innovative strategies to improve the quality of care for Latinos.

Cigarette Smoking Cessation and Prevention.
We have demonstrated that Latinos differ from Caucasians in smoking patterns and acceptance of standard cessation mechanisms. We found that many Latinos are light and intermittent smokers and that they are less likely to accept pharmacologic treatment (#131 and #137). Building on our descriptive research, we have developed and tested innovative culturally appropriate smoking cessation programs. We found that web based cessation intervention in Spanish and English showed abstinence rates of 15% to 24% at one year, comparable to quit lines and nicotine replacement (#125). We have also studied smoking patterns in Argentina among adults and youth. With a R01, we conducted a prospective cohort study to characterize smoking patterns among 3500 mostly indigenous youth living in Northwest Argentina. In our competing renewal, which will be funded by the National Institute of Drug Abuse, we plan to test a community-based smoking prevention intervention targeted at youth in two northwestern provinces and a physician-mediated smoking cessation intervention for adults in Buenos Aires. Through this project we have also analyzed tobacco documents relevant to Argentina and identified industry subversion of anti-tobacco legislation (#124), obstruction of litigation efforts (#133) and marketing strategies directed at youth (submitted).

Cancer Screening and Prevention.
With NCI funding, we have studied the impact of uncertain mammography interpretations on women, finding problems with communication, patient misunderstandings, and an association with significant depressive symptoms. We have found that vulnerable populations (Latina immigrants in #118) and jail inmates in #119) frequently do not receive recommended breast and cervical cancer screening. We are currently studying how risk perception of diverse women affects obtaining cancer screening tests, use of a drug to prevent breast cancer and decisions to stop Pap smears and obtain a test for HPV.

Minority Aging Research.
The Center for Aging in Diverse Communities (CADC) is an NIA supported P30 center. CADC supports investigators from diverse ethnic backgrounds and conducts methodological studies applicable to aging in diverse communities. CADC funded investigators published on patient perception of how cultural factors affect quality of care (#114), genetic admixture of Latinos with asthma and response to treatments (#117 and #136), how Latinos contribute to the role of race and genetics, (#128), the epidemiology of dementia in different ethnic communities (#135), the role of language and specialty care in determining the quality of care of patients with diabetes (#138), and the efficacy of a church-based intervention to promote use of adult immunizations (#141). We are currently conducting an evaluation of a video medical interpretation technology for limited English-proficient patients in both inpatient and ambulatory care settings. CADC has made methodological contributions concerning recruitment of diverse participants, translation guidelines, measures of acculturation, perceived discrimination and social class. CADC was competitively reviewed for a third 5-year cycle and received the highest priority score (128) and funding will start before October 1. The new CADC scholars will develop and test a cognitive assessment battery in Spanish, study vaginal symptoms in older women and the role of racism in health status among older African Americans.
